Worker killed in plant blast

GURUN: An employee of a Petronas Fertilizer (Kedah) Sdn Bhd contractor was killed in a blast while carrying out maintenance work at the Petronas ammonia plant here yesterday.
Zaidi Selamat, 32, from Shah Alam, who worked for Stork Cooper Heat (M) Sdn Bhd, suffered head injuries when he was hit by a check valve’s sealing clamp that flew off during the blast at 12.45pm.
Petronas Fertilizer’s media relations department confirmed the tragedy.
In a statement, it said that Zaidi was sent to Sungai Petani Hospital, but succumbed to his injuries.
No other casualty was reported although there were other contract workers near the plant at the time.
The statement said that Petronas shut down the plant following the incident to allow the authorities and police to carry out investigations.
It is learnt that Zaidi had been carrying out maintenance work at the plant for the past week.
Kuala Muda OCPD Assistant Commissioner Hashim Ali said police had classified the case as sudden death resulting from an industrial accident.
“We’re still investigating the cause of the accident,” he told reporters outside the plant.

Hmmm…. What cud I say. Working in a process plant is a very high-risk job. I believe everybody knows that. A slightest mistake will cost u ur life. Even after all the precautions that have been taken prior to start work, u will never know what cud happen. Like this unlucky guy. I still didn’t get the whole story yet, but I believe and sure few peoples will be in a deep shit. This case will be investigated by many authority parties. DOSH is for sure. Plant Manager, Shift Supervisor, operators, safety officers, Contractor's supervisor, etc. especially those who signed the permits. That’s why many people would happily sign Cold Work Permit but very reluctant to sign Hot Work Permit. Coz Hot Work Permit means high risks. I always remind myself to be extra caution when performing any Hot Work at the plant area.
After the last death incident at Kertih in 2002/2003 (the guy mati tergantung terbakar, and others cud only watched him burnt to death helplessly due to his position at the moment), the rules are more strict. Petronas has reviewed and revised all their rules and regulations on working in a live plant. But who can predict what will happen in the future, rite? We as a human being cud only miminise the potential hazard, but we could not eliminate the risk at all. Anyway, we could take all the necessary precautions and be alert of our surrounding. We as the contractors / workers, have the right to stop work if we feels that it is not safe to continue doing the works until the situation has been assess again and confirm safe to work. I always stress that part to my workers. Nyawa dulu. God grant us just one chance to live our lives to the fullest. Pentingkan keselamatan sendiri, don’t leave it to others to take care of your safety. And sometimes gerak hati is important. Follow ur hunch.
So far as I know, in the death incident at Kertih, few people / staffs got fired. Plant Manager has also been changed. Till that extend, huh! Well, it was a lesson learnt for all of us working in the live plant / process plant. Tak boleh main2 masa kerja. Now, if u are to work in a Petronas plant, u have to attend a course (NPSP) and sit for an exam. If u pass, u will get a Passport to work in any Petronas Plant. And of course u have to pay for it. Quite costly. RM250. Kalu company kecik2 mati lor..... And again prior to start work, at every different plant, u need to attend the safety induction. Some of the plant u have to sit for their exam pulak. Bukan senang. So for those yang buta huruf tu, memang susah ler. And those who wants to apply the PTW (permit to work) / work leader, also have to attend a different course and pass the exam. Ingatkan masa sekolah jer ada exam, dah keje pun ada jugak.
During the safety induction, they will show us few incidents that happened all over the world relating to process plant / offshore. Ngeri gak wei! And I tell u now, i saw one video showing the explosion of a tank I can't remember where, in just a few seconds, puff..... habis lenyap a few kilometres of the area. Can't imagine if the same thing happen in Kertih, where only one specific tank (cannot tell u la which one) explode, cud sweep away the entire 30km radius of the surrounding. Habis Kemaman tu...... and Dungun as well.
Hmmm.... about the latest incident, maybe it has something to do with our attitude as well. When an incident just occur, we will take extra precautions, but then after a few years, we started to ease a bit, and tends to forget few things. Normal human being. If it is true due to human error, that person will forever bring the guilt to his grave.
For the victim, let us pray for his soul to rest in peace. Al-fatihah.
